  • Lenoir, NC

Job Description

Description:

Help more patients and families receive the care they need. AMOREM is seeking a Clinical Liaison who combines strong nursing skills with the ability to build trusted relationships, educate others, and coordinate care during some of life’s most important moments. This role connects hospital providers, patients, and families with AMOREM’s hospice services. As the primary contact for assigned hospital, the Clinical Liaison helps healthcare partners recognize when hospice may be appropriate, supports informed decisions, and coordinates timely transitions to care. The ideal candidate is a confident clinician, a clear communicator, and a proactive relationship builder who takes ownership of follow-through. Your work will help more patients and families access the quality, thoughtful, loving care at the heart of AMOREM’s mission.

Requirements:

Essential Duties and Responsibilities

  • Serve as AMOREM’s primary liaison and organizational ambassador within assigned hospital.
  • Develop and maintain strong relationships with physicians, case managers, social workers, discharge planners, nurses, and other healthcare professionals.
  • Provide education about hospice, AMOREM’s services, and the benefits of timely referrals for eligible patients.
  • Serve as an end-of-life care resource for hospital staff, patients, and families.
  • Identify barriers to referrals and transitions and work with hospital partners and AMOREM teams to resolve them.
  • Help patients and families understand hospice services and available care options with compassion, clarity, and respect for their goals.
  • Complete nursing assessments and document findings accurately and promptly in AMOREM’s electronic medical record and hospital systems, as required.
  • Collaborate with hospital providers and AMOREM’s interdisciplinary team to support an individualized, patient- and family-centered plan of care.
  • Coordinate hospital referrals and transfers for routine hospice care and general inpatient hospice care, including transitions to home, long-term care facilities, or AMOREM patient care units, as appropriate.
  • Visit hospitalized hospice patients to support continuity of care and communication among patients, families, hospital staff, and the hospice team.
  • Respond promptly to referrals, questions, and requests, and follow through until needs are addressed.
  • Provide responsive, professional service to patients, families, hospital partners, and colleagues.
  • Demonstrate sound clinical judgment, strong problem-solving skills, and the ability to manage competing priorities.
  • Uphold AMOREM’s mission, vision, and values through daily interactions and job performance.
  • Participate in Quality Assurance and Performance Improvement activities.
  • Perform other related duties as assigned.

Qualifications

  • Bachelor’s degree in nursing.
  • Minimum of two years of nursing experience; hospice experience strongly preferred.
  • Strong clinical assessment, communication, and organizational skills.
  • Ability to build collaborative relationships with healthcare professionals and communicate sensitively with patients and families.
  • Ability to work independently, anticipate needs, and coordinate effectively across teams.
  • Commitment to compassionate care, professional integrity, and expanding access to hospice services.

Bring your clinical expertise to work that matters. Join AMOREM and help ensure more patients and families understand their options, receive timely support, and experience care that honors their needs and wishes.
